• Appointment letter from the United States Embassy
• Passport
• Passport-sized photograph (one for each applicant, including babies)
• Medical questionnaire, form IV18a (one for each applicant, including babies)
• Police certificate (unless you have already sent original to the Embassy)
• Spectacles or contact lenses if worn
• Vaccination records, with original certification
• Children's 'Red Book' health record
• Medical reports of any past or current illnesses including psychiatric. If there is a history of TB, please obtain old records, including investigation and treatment details
• When there may be special educational needs, please provide any recent educational, psychological assessments, school reports etc.
